Please answer all COVID screening questions honestly and accurately.
Lakeside Medical Clinic’s team is trained to screen every patient for symptoms of COVID19, and for risk factors such as travel and recent exposure (or potential exposure).
If you do have symptoms of COVID19, have travelled in the last two weeks, or have been exposed (or potentially exposed) to someone who has tested positive for COVID19, you will be asked to either cancel, reschedule or switch your appointment to a telephone assessment.
We understand that this policy is inconvenient, but it would not be a policy if it was not extremely important. The reason for our thorough COVID19 screening process, is to protect our staff, our physicians and our patients- many of whom are immunocompromised.
By adhering to this policy, you are helping us keep everyone who enters Lakeside Medical Clinic safe- and potentially saving lives.
